Candidate Agreement

1. I certify that the accuracy of all information I provide both on this application and in a resume is correct. I understand that any false statements may cause Equis, LLC to refuse, or lose my employment.

2. I have submitted this application for the purpose of Equis, LLC in assisting me in securing employment (temporary, consulting or full-time). I understand that there are no fees to candidates associated with this process as well and there is no guarantee that employment will be obtained.

3. I approve Equis, LLC to conduct background checks for employment purposes. A background check may include, but not be limited, to employment and educational history verifications. I release employer’s, school and other persons from liability in responding to inquires connected with my application and specifically authorize the release of information by any institutions, companies, individuals or parties listed on any interaction obtained to a client company.

4. As a registered candidate of Equis, LLC I may choose to participate in testing assessments or certificates. I release Equis, LLC to communicate results to client companies and to not hold them liable for the release of any data.

5. Given Equis, LLC obtains employment for me as a contractor or temporary, I understand the following terms apply:

a. I am employed Equis, LLC and not the client company I work for,

b. I understand my employment at a client company is “at will” and can be terminated for any reason at any time by either party.

c. I understand that should a client company offer me employment with their company, either directly or through another company, I am to notify Equis, LLC immediately, due to the fact that the client company has an obligation to pay Equis a fee. This arrangement is in place for one year from my last day of employment at the client company.
 
d. I understand that all my work is supervised by an employee of the client company. I do not have the authority to sign documents for a client company nor as a representative of Equis, LLC.
